CMS Home Care Guidelines
Medicare has set forth the following criteria for home health admissions:   • Patient to whom the services are provided is an eligible Medicare beneficiary • Under the care of a physician • Patient must have physician’s orders. • Patient must be homebound:   • the patient may leave home infrequently; however, leaving home would be considered a taxing effort • the absences from home are primarily attributable to the need to receive medical treatment • Patient must require the intermittent services of a skilled nurse and/or a physical therapist. The home health service is a necessary and reasonable treatment of illness or injury, which cannot be performed by someone other than a skilled health care professional. PLEASE NOTE: The following items are required prior to home health agency performing patient care: 1. Demographics. 2. History and Physical. 3. Current Medication Profile 4. Discharge Orders.
