CMS Home Care Guidelines  
Medicare has set forth the following criteria for home health admissions:
 
Patient to whom the services are provided is an eligible Medicare beneficiary
Under the care of a physician
Patient must have physician’s orders.
Patient must be homebound:
 
the patient may leave home infrequently; however, leaving home would be considered a taxing effort
the absences from home are primarily attributable to the need to receive medical treatment
Patient must require the intermittent services of a skilled nurse and/or a physical therapist.
The home health service is a necessary and reasonable treatment of illness or injury, which cannot be performed by someone other than a skilled health care professional.

The following items are required prior to home health agency performing patient care:
1. Demographics.
2. History and Physical.
3. Current Medication Profile
4. Discharge Orders.
